The following sections describe the workflow for how various forms that require Department Chair and/or Dean level approval are routed in the College of Sciences. It is important for students and faculty to follow these processes to ensure timely review and approvals. Please note that it may take 5-7 business days for decisions to made on undergraduate forms after it has reached the department.

All documents that are routed through the College of Sciences for review must be saved as PDFs. Students should complete any PDF forms electronically using Adobe Acrobat. Requests can only be received from your UTSA email address.

Student Initiated Forms

Student-Initiated Forms requiring Department Chair and Dean level approvals must be routed through the student's department. These forms include Independent Study, Internship, Permission to Enroll in Graduate Courses While an Undergraduate, and COS Undergraduate Thesis. The COS Dean's Office can only receive these forms through the department.

Students can submit their forms to the department through the department's undergraduate program manager. Students must ensure that their forms are complete and have all required documentation attached. Forms requiring instructor and/or academic advisor approval should already have their signatures prior to submitting them to the department. If applicable, students can ask their instructor to send the form directly to the department to start the review and approval process.

Often, these forms are related to a class. Students should route the form through the department that houses the class regardless of their major.

If the form is approved by the Department Chair and Dean's Office, the undergraduate program manager will send the signed form back to you. Students will need to then upload the form along with all required documentation to the Document Uploader.

These forms should be entered into the approval workflow at least one week before the start of classes to avoid any delays in registration.

Advisor-Initiated Forms

Advisor-Initiated Forms requiring Dean level approval, such as Major/Minor Course Substitution, Core Substitution, and Waiver of University Requirement petition forms, can only be submitted to the Department or College by the student's academic advisor. The COS Dean's Office and its academic departments can only receive these forms through the student's academic advisor.

For Major/Minor Course Substitution petition, it is recommended that students speak with their major's Undergraduate Advisor of Record to discuss the request. If the UGAR agrees with the request, then the student can ask their advisor to create this petition.

It is important that the student provide as much documentation to their advisor as possible (e.g., syllabi, justification letters, transcripts). Consult with your advisor on what documentation is needed.

Other Forms

For forms only requiring the COS Dean level approval, students can submit directly to cos.undergraduate@utsa.edu for review. Note that students are still responsible for securing any signatures that are required before the Dean level signature.

These forms may include Petition to Attempt a Gateway Course, the COS Request for Early Reinstatement, Student Petition for a Course Exemption to the Six-Drop Policy, and appealing a department chair decision on an Academic and Grade Grievance.
